  • We believe that your voices are key to improving our mental health services. Our aim is that young people are involved in all aspects of our work.

    Become a Young Advisor at HFEH Mind. By getting involved, you will meet like-minded people and gain skills, experiences, and a unique opportunity to add to your CV.  If you contribute to two or more involvement activities per month, you will be rewarded for your time with monthly gift vouchers.

    To become a Young Advisor, you must be

    • Aged 10-19
    • Live in Hammersmith, Fulham, Ealing and Hounslow, or;
    • Go to school or college in Hammersmith, Fulham, Kensington, Chelsea, or Westminster
    • Be passionate about creating positive change in mental health for young people 

    To sign-up, please fill out the details below. If you are under 16, we will need consent from a parent or carer before you can get involved.

    We will need to keep in touch with you about the participation activities you can get involved with. We will generally email you but may occasionally phone you as well.

  • DATA COLLECTION STATEMENT 

    To facilitate Youth Participation activities, including the Hammersmith, Fulham, Ealing, and Hounslow Mind (‘HFEH Mind’) Youth Panel, we need to store personal data relating to the children or young people we work with.

    Before agreeing to be involved in Youth Participation activities at HFEH Mind, we must ensure you understand and agree to the way we store personal data.  

    We process and store personal data in compliance with the Data Protection Act 2018. All the personal data you provide to us will be stored internally on secure databases. This personal data is only accessible to HFEH Mind staff.

    In line with our Data Retention Policy, we will retain the personal data collected on our secure databases for a period of up to six years unless you instruct us otherwise. After the six-year retention period, the personal data collected will be safely and securely erased from our databases.

    Individuals have legal rights regarding data processing, and the personal data that is recorded about them, including the right to request access to the personal data that HFEH Mind holds on them.

     

     

    DATA SHARING STATEMENT 

    Your personal data will not be shared outside our organisation unless:

    • You provide clear consent for specific information to be shared with a specific third-party individual, or organisation, for a specific purpose.
    • We have a safeguarding concern, i.e., we are concerned about serious harm or life-threatening risk to you or others, or we are concerned about indications of abuse or neglect.
    • We have a legal obligation to do so. 

    We will never share personal data for marketing purposes.

    This consent form should be completed by the young person aged 16 or above.

    This consent form should be completed by a legal Parent / Carer of the young person who is under the age of 16.
